Phyllis Mae Montgomery Cook Casstevens, 77, passed away Tuesday September 1, 2015. She was born August 11, 1938 in Stokes County to the late Franklin Lee and Hazel Mozelle Pratt Montgomery. She was a member of Mitchell's Chapel United Methodist Church. She was preceded in death by her parents, her first husband Worth Edward Cook, Sr., her second husband George Hubert Casstevens, a brother Olen Montgomery. Surviving are her children Xiena (Charbel) Farah, Kathy (Kelly) Pfouts, Joe (Peggie) Cook, Angela Schmidt, Worth (Susan) Cook, Jr., Audra (Mark) Tyler; eleven grandchildren, three great-grandchildren; sisters Peggy Hall, Sara Mecum, Pauline Ellis; a brother Lee Montgomery. A private graveside service will be held at a later date. The family would like to say a very special thanks to the staff at Hugh Chatham Hospital and the staff at Wake Forest Baptist Medical Center for the loving care given to Mrs. Casstevens.
